Home-health reference: source and preparation record

The first ACD home-health edition connects agencies to served ZIP codes, care-quality measures and patient experience for provider research.

18quality measures per agency
12survey measures per agency

Source edition and reporting periods

All five CMS datasets were released July 15, 2026. Agency, ZIP and quality-period metadata were modified May 27; survey metadata June 10. The retained dictionary is version 14, April 2026. ACD captured and prepared this edition October 18 UTC. OASIS measures cover October 2024–September 2025; HHCAHPS covers calendar 2025. Discharge-to-community and Medicare spending cover 2023–2024, preventable readmission 2022–2024, and preventable hospitalization 2024. Quality stars combine underlying periods.

The ZIP file reports areas served, but these retained sources do not specify its exact collection dates. Its relationships are not assigned the quality or survey dates and do not establish current availability.

Preparation and coverage

All 12,460 agencies have corresponding survey and ZIP records. The eight tables retain every source relationship, including 565,608 agency/ZIP pairs across 34,842 ZIPs. The detailed tables contain 224,280 quality observations and 149,520 patient-experience observations. Every one of 590,551 source CSV rows is accounted for. Identifiers remain text; source dashes in agency services remain unavailable, not No.

Care-quality and survey ratings remain separate. Numerical values retain original cells, precision, explicit missing statuses, reporting periods and CMS footnotes. Counts with thousands separators become usable numbers. Published values can still carry qualifications, such as a small survey response count.

Source values requiring interpretation

Risk-adjusted percentages are not reconstructed from raw numerator and denominator counts. Thirteen published discharge-to-community values exceed 100; 129 records have a value or bound above 100. These are preserved and flagged, not clipped or interpreted as ordinary patient shares. The Medicare spending measure is a ratio to a national benchmark, not dollars. No agency ranking or clinical recommendation is added.

Release identity and included improvements

This is the first ACD release of these retained CMS inputs, not a source-change comparison. Independent acceptance checks the original CSV cells, period mappings, footnotes and coverage. Raw files, lossless staging and preparation recipes reproduce the publication offline. Processing improvements for this purchased source edition are included; a future paid source edition requires separate entitlement review. No subscription, scheduled refresh or automatic revision-email implementation is claimed.
