01 / KNOW THE FIT
A useful join starts
with a clear meaning.
What you can do with it
Start with a ZIP-code market list or CMS agency identifiers. Identify historical service relationships, inspect reported services, and compare the same quality or survey measure across agencies. Useful for healthcare market research, provider-network analysis and consulting.
What one row means
agencies.csv has one row per CMS Certification Number (CCN). served_zips.csv has one row per agency and served ZIP. quality_measures.csv and patient_experience.csv have one row per agency and measure, including unavailable observations.
Unique keyagency_id
How to join it
Import CCNs and ZIPs as text. Join served_zips.csv to your postal ZIP list, then agencies.csv on agency_id. Multiple agency matches are expected. Filter measure_id before adding a long measure table. The office ZIP is different from served ZIPs; neither is a Census ZCTA or county FIPS.
Coverage, with the edges visible
All 12,460 agencies in the retained CMS file, with matching survey and ZIP-reference agency universes. Includes all 565,608 agency/ZIP pairs, 18 quality measures and 12 survey measures per agency. All 590,551 rows across five source CSVs are accounted for without dropping or deduplicating records. Covers 50 states, DC and the source territories PR, GU, MP and VI.
Keep in mind
- ZIP relationships describe areas where an agency served at least one patient during the source collection period. Exact ZIP-table dates are not supplied. They do not guarantee current service territory, current availability, or patient volume.
- Services are agency-level flags, not ZIP-specific services. Source dashes remain unavailable, not No. This is a dated source universe, not an independently verified active-agency list.
- Quality stars and patient-survey stars are separate measures. Survey results nominally cover 2025; notes can qualify the period or response count. Quality and claims periods vary. These data are not individual care recommendations.
- Risk-adjusted rates need not equal raw numerator divided by denominator. In 129 discharge-to-community records, a published value or bound exceeds 100; these are preserved with a flag, not clipped. Spending is a ratio to a national benchmark, not dollars.
- No patient records, current availability, NPI/county-FIPS crosswalk, geographic allocation weights, HHVBP payment-performance data or state/national aggregate benchmark tables are included.
Preserves source identifiers, every agency and every ZIP relationship. Separates 18 care-quality measures from 12 patient-survey measures, maps reporting periods, retains counts and bounds, resolves survey footnotes and distinguishes missing numbers from zero. Source dashes in service flags remain explicit. Independent acceptance checks original CSV cells and dictionary text; retained raw, staging and preparation recipes reproduce offline.
