Hospital reference: source and preparation record

The first ACD hospital reference separates facility attributes from patient-experience observations, ready for a CMS-ID research portfolio.

Source release and observation dates

CMS labels Hospital General Information and Patient survey (HCAHPS) - Hospital released August 13, 2026 and modified July 22. ACD captured them October 17. The survey observations describe October 1, 2024 through September 30, 2025. Overall hospital ratings combine measure-specific periods and do not share that one survey period.

This is the first ACD release of these retained inputs. There is no previous ACD release to compare, and no claim that CMS issued these data in October.

ACD preparation

The facility table has one row per CMS facility ID. The separate survey table has one row per facility, measure and period. Preparation preserves all source records, validates their keys and parent relationships, keeps alphanumeric IDs and leading zeros, standardizes dates and resolves footnote codes. Numerical fields retain distinct published, unavailable and inapplicable statuses beside their original cells.

Coverage and interpretation

Of 5,419 facilities, 3,174 have published overall stars and 2,245 do not. The survey file contains 68 measure records for each of 4,790 facilities, including unavailable results; 629 facility records are absent from that file. Missing survey membership is not a zero score. Footnotes can qualify scores that are published.

Overall stars differ from patient-survey stars and answer percentages. Survey counts and response rates repeat across measure rows. The download does not include NPI or county-FIPS mapping, individual patients, or all underlying CMS quality measures.

Reprocessing and purchase

Retained raw, lossless staging and prepared layers reproduce offline. An independent acceptance pass compares every delivered source-derived value, status, date and footnote with the original files. Processing improvements on these inputs are included for buyers. A new source edition may be sold separately after entitlement review; no refresh subscription is promised.
