Nursing-home reference: source and preparation record

The first ACD nursing-home reference combines facility attributes with clearly defined capacity, rating and staffing measures for a CMS-ID research portfolio.

14,690facility records
308,490facility / measure records
21selected measures per facility
14,565published overall ratings

Source edition and measurement periods

CMS released Provider Information and Data Collection Intervals October 8, 2026; metadata was modified September 30. Both files retain a September 1 processing date. ACD captured and prepared this initial edition October 18 UTC. Staffing levels cover January–March 2026; turnover covers 2025. Ratings combine their own underlying periods, which are not replaced with the staffing quarter.

What preparation adds

All facility identifiers remain text, including 247 Medicaid identifiers containing letters. The main table has one row per facility; the detailed table has one row per facility and selected measure. Numerical values retain their precision, original cells, explicit statuses, units and resolved CMS footnotes. Two official collection intervals supply the staffing and turnover dates.

Coverage and accounting

All 14,690 providers are retained. There are 14,565 published overall ratings and 125 missing overall ratings. All 21 selected measures are represented for each facility, including unavailable values. The source-row audit accounts for 14,737 rows: all providers and 47 interval records. Two intervals are included; 45 intervals for other measures are explicitly excluded. Unselected provider columns remain in raw/staging and are listed in the quality report.

Interpreting the fields

Published values may still carry footnotes. Missingness never becomes zero. Reported staffing ratios differ from case-mix-adjusted ratios and employee headcounts. The overall rating is not a simple average of component stars; inspection ratings use state-relative methods. An SSA county code is not Census county FIPS. This package does not supply a county/NPI crosswalk, individual patients or complete inspection and quality-outcome histories.

Reprocessing and future editions

Retained raw, lossless staging and processing recipes reproduce offline. Independent acceptance compares every delivered source-derived field, period, status, footnote and source-row outcome with the original sources. This is the first ACD edition, with no prior-release comparison. Processing improvements on these inputs are included for buyers; future paid source editions require entitlement review. No subscription or refresh schedule is included.
