01 / KNOW THE FIT
A useful join starts
with a clear meaning.
What you can do with it
Enrich a CMS-ID facility list for provider research, healthcare market analysis or consulting. Compare the same measure and period across facilities, using beds and provider attributes as context and source footnotes to qualify the results.
What one row means
facilities.csv has one row per CMS Certification Number (CCN). facility_measures.csv has one row per facility and measure: all 21 selected measures, including missing observations, with units, periods, original values and footnotes.
Unique keyfacility_id
How to join it
Import CCNs and ZIP/SSA county codes as text, preserving zeros and letters. Join facilities.csv directly to a CCN list. Select measure_id before joining the detailed file for one observation per facility; joining all measures produces 21 rows per facility. An SSA county code is not a Census county FIPS code.
Coverage, with the edges visible
All 14,690 facilities in the retained CMS Provider Information file, including 247 letter-bearing Medicaid identifiers. Of these, 14,565 have published overall ratings and 125 do not. The detailed table contains 21 measures for every facility. All 47 source interval rows are accounted for: two staffing intervals included and 45 other-measure intervals excluded.
Keep in mind
- This is a fixed CMS edition, not a live directory. Names, ownership categories, chain labels and flags are retained as published without independent current verification.
- Staffing levels cover January–March 2026, turnover covers 2025, and ratings combine their own underlying periods. CMS release, source processing and ACD preparation dates differ.
- Overall, staffing, quality and inspection stars are different measures; overall stars are not a simple mean. Inspection ratings use state-relative methods. These data do not establish which facility is appropriate for an individual.
- Reported staffing ratios are hours per resident day, not headcounts. Adjusted ratios are CMS comparison measures, not actual hours. Missing values remain distinct from zero, and footnotes can qualify published scores.
- No NPI/county-FIPS crosswalk, patient records, individual owner details, raw payroll, complete inspection history or underlying MDS/claims quality outcomes. Selected facility flags and most recent survey date are included; numeric inspection and penalty details are outside this product.
Preserves all provider rows and source identifier text; checks unique facility and facility/measure keys; separates published numbers from missing values; maps staffing and turnover to the official collection intervals; resolves CMS footnotes; and accounts for every source row. Independent acceptance compares delivered cells with original CSVs and the dictionary. Retained raw, lossless staging and preparation layers reproduce offline. How we prepare and version our data →
